When you come to the Centre for Driving Ability and Vehicle Adjustment (CARA), you need to take some documents.
Register yourself at CARA
Download the medical questionnaire and send it back to CARA duly completed.
Please fill in the last page of the medical questionnaire carefully to enable us to take into account your preferred location for an appointment.
Documents for physicians
- You are a physician and you want to / are entitled to make a decision on the ability to drive yourself?
Download model VII certificate and fill it out. - You are an ophthalmologist and you want to / are entitled to make a decision on the ability to drive yourself?

Documents for driving schools
You run a driving school and you wish to reserve a CARA vehicle?
